How to make a PDF fillable in Adobe Acrobat
Adobe's tool for this is Prepare a form. In the current Acrobat desktop app it sits under All tools. Older versions list it as Prepare Form under the Tools tab. You select a PDF, another document or a scan, and Acrobat analyzes the page and adds fields where it finds blanks. You then add any it missed, rename fields and save.
Prepare a form is part of the paid Acrobat Standard and Acrobat Pro apps. The free Acrobat Reader fills forms and places text with Fill & Sign, but it can't add form fields. In the US, Adobe lists Acrobat Standard at US$14.99 a month and Acrobat Pro at US$19.99 a month on an annual plan billed monthly, as of October 2026.
Automatic detection saves time on long, cleanly typed forms, but it guesses. It can give fields names like Text1, miss tick boxes drawn as shapes or turn two blanks into one field. Review every field before you send the form out.

Create a fillable PDF from Word or Google Docs
Word has its own form controls on the Developer tab, such as text boxes, check boxes, date pickers and dropdown lists. They work inside Word. Word's Save as PDF doesn't turn them into PDF form fields, so the PDF you get is flat. A PDF downloaded from Google Docs is flat too, and Google Forms makes a web form, not a PDF.
So build the layout in Word or Docs and add the fields afterwards. Type each blank as a run of underscores, like Name: ________, then save as PDF. Most failures trace back to one of these causes.
