Create Fillable PDF Form, free and private

To make a PDF form fillable, add the PDF here, pick a field type and click or drag on the page where people should type, tick or choose. Name each field, then press Create fillable PDF and download the new file. You can add text boxes, paragraph boxes, checkboxes, radio buttons, dropdowns, date fields and signature lines.

The fields are standard AcroForm fields, the form format that Adobe Acrobat Reader, Chrome, Edge, Firefox and Preview on a Mac can all fill. Your PDF is turned into a fillable form in your browser and is never uploaded.

How to make a PDF form fillable

  1. 1

Add the PDF you want to make fillable

Press Select a PDF file or drop it onto the page. It can be a form saved from Word, a printed form you scanned or any other PDF. The pages appear as a large preview.

  2. 2

    Place the fields

Pick Text, Paragraph, Checkbox, Radio, Dropdown, Date or Signature, then click the page to drop a field or drag to draw its size. Drag a field to move it and drag its corner to resize it. Press Ctrl+D (Cmd+D on a Mac) to duplicate the selected field and Ctrl+Z (Cmd+Z) to undo. To fill every typed blank at once, press Add text fields on ____ lines.

  3. 3

Name and set up each field

Click a field to open its settings. Give it a clear name, add a default value or tooltip if needed, set the text size and turn on Required for answers that must be given. Dropdowns take one choice per line.

  4. 4

    Create and download

Press Create fillable PDF. The fields are written into the original PDF, which keeps its pages, text and links. Press Preview to try the form, then Download fillable PDF. The file name ends in -fillable.

Why use PDFToolz for this

Every common field type

Single-line text, multi-line paragraph boxes, checkboxes, radio button groups, dropdown lists, date fields and signature lines. Each field can have a default value, a tooltip, a required flag and its own text size.

Works in the apps people already use

Each field is saved with a drawn appearance, so it shows correctly before anyone clicks it. Acrobat Reader, browsers and Preview can fill and save the form with no extra software.

Finds typed blanks for you

Blanks typed as underscores, like Name: ________, become text fields with one click. You only have to check and rename them.

No upload

JavaScript on your device reads and changes the PDF. HR, medical and client forms stay on your computer, and there is no account or watermark.

How to make a PDF form fillable, step by step

Start from the finished document, with its labels, lines and boxes already in place. Making it fillable only adds fields on top of the page. Fix spelling and layout first in Word or Google Docs, then save the file as a PDF.

Add the PDF and work down each page. For a typed answer, pick Text and drag over the line where the answer goes. Use Paragraph for comments and addresses that need several lines. Leave Text size on Auto when the box is small, so the viewer shrinks long answers to fit. A fixed size such as 10 or 11 points looks tidier on long forms.

When every blank has a field, click through the list of fields on the right and give each one a clear name, like First name or Start date. People never see the names, but they appear in exported form data and help anyone who edits the form later. A tooltip, such as Enter your date of birth, is shown on hover and read out by screen readers.

Checkboxes, radio buttons and dropdowns

Use a checkbox for a single yes or no answer, like I agree to the terms. Use radio buttons when people must pick exactly one answer from a short list, like Yes, No or Not sure. Use a dropdown for longer lists, like countries or departments.

Radio buttons work in groups. Place the first button, then keep it selected and click next to each of the other answers. Every button placed while a radio button is selected joins the same group, so picking one clears the others. To start the next question, press Esc so no button is selected, then place its first button. Give each button a value, such as Yes or No. The value is what the form stores, while the printed label on the page is what people read.

  • A checkbox is one tick box on its own. Turn on Checked by default to start it ticked.
  • A radio group takes one answer from a few. Each button needs its own value.
  • A dropdown takes one answer from a long list. Type one choice per line and pick a default if you want one.

Date and signature fields

A date field is a text field sized for a date, with the tooltip Date (MM/DD/YYYY). Change the tooltip to DD/MM/YYYY or another format if your readers expect one. The format is only a hint. Nothing checks what people type, because date checks need scripts that only Acrobat runs.

A signature field is a larger text line where the person types their name, Many offices accept a typed name as a simple electronic signature. It is not a certificate-based digital signature. To sign the form yourself with a drawn or uploaded signature, fill it first and then use eSign PDF.

Create a fillable PDF from a scanned or Word form

A scanned paper form is only a picture of the page, but you can still place fields on top of it. Add the scan, draw a field over each blank and download. Find blanks needs real text. On a scan, run OCR PDF first or draw the fields by hand.

Forms saved from Word or Google Docs often use typed underscores for blanks. Press Add text fields on ____ lines and a text field is placed on each run of three or more underscores. Lines drawn as shapes or table borders are not detected, so add those fields yourself.

Tips for a form that is easy to fill

These settings make a fillable form quicker to fill and easier to read.

  • Make each box a little taller than the printed text, so answers are easy to read and click.
  • Choose No border when the page already has printed lines or boxes, and Thin border on a blank page.
  • Turn on Required only for answers you must have. Acrobat and some browsers outline required fields, but they don't block saving.
  • Give every field its own name. Fields that share a name also share one answer, so the tool asks you to rename them. Radio buttons in one group are the exception.
  • Test the result in Preview before you send it, then compress it with Compress PDF if it is going to an upload portal.

If the form does not behave as expected

Most PDFs work straight away. If something looks wrong, the cause is usually one of these.

  • The PDF already had fields. They are kept as they are. A new field whose name is already taken gets a number added. The result says how many were renamed.
  • Fields are missing in Acrobat. The file may have been an XFA form, an older Adobe format that hides standard fields in Acrobat. The XFA part is removed when you create the fillable PDF, and the result says so.
  • Accents look wrong in a default value. Default values with accents, Greek or Cyrillic are drawn with a Unicode font. Chinese, Japanese and Korean are not supported.
  • The PDF asks for a password to open. Remove it with Unlock PDF, using the password you know, then make the unlocked copy fillable.
